Thank you in advance for your patience. Forms must be filled out the day of your appointment / within 24 hours or will be considered null and void. If you would like to print and complete prior to appointment, kindly make them 2 sided 1 page copies. I also have copies available at the office. 

For new clients you will need to complete the New Client Form + Health Information & Informed Consent Form (2 forms total)

For existing clients you will need to complete the Health Information & Informed Consent Form for every massage appointment. (1 form total)
